Battle Ground Public Schools Immunization Requirements, 2011
Washington state law requires certain immunizations against childhood diseases for all students, regardless of age, who attend a public or private school or a licensed day care center.
On or before the first day of attendance, proof of the child's
(a) full immunizations,
(b) initiation of schedule of immunizations, or
(c) certificate of exemption, current with the 2011-2012 state requirements*
must be presented at the school. A completed and signed Certificate of Immunization Status (CIS) is required to be on file at the school, preschool, or child care facility. Below are the vaccines required for school attendance.

Changes to School and Child Care Immunization Exemptions, 2011*
Effective July 22, 2011 Washington State Law requires a licensed health care provider to sign the Certificate of Exemption in order for a parent or guardian to exempt their child from school and child care immunization requirements. The signature verifies that the health care provider has provided the parent/guardian information about the benefits and risks of immunization. A signed letter from a health care provider stating the same information is also acceptable.
This law only applies to exemptions requested after the July 22, 2011 date. Parents/guardians who demonstrate membership in a church or religious group that does not allow a health care provider to provide medical treatment to a child only need to complete the Certificate of Exemption form, Box 2, in order to be in compliance.
