It's Kindergarten Registration Time!


Register at your neighborhood primary school
9:30 a.m. - 3:15 p.m., April 23-27
 In addition:  Yacolt Primary will have an Open House March 29th 5:30-7:30 p.m.
Glenwood Heights Primary will have a Kindergarten Roundup for parents April 24th 1:30-2:30 p.m.

If you do not know what school your child will attend call the District Boundaries Office at 885-6577


 


Here is what you need to register your child:
Verification of birth date (birth certificate). Child must be five years of age before September 1st.
Immunization Record (complete)
4 doses of DTaP with last one on or after the fourth birthday
4 doses of Oral Polio if all doses are given before the fourth birthday - 3 doses if the last dose  is given on or after the fourth birthday
 2  doses of MMR (first dose on or after the first birthday, second dose at least 28 days after the first dose, prior to starting school)
 3 doses of HEP-B
2 doses of Varicella (chickenpox) received on or after child's first birthday and at least 28 days apart.  Parent-reported history of disease is NOT acceptable.

Schools must receive complete student information and up-to-date immunizations prior to class placement.
